Get them to first write a bullet list of the pints they want to cover, this will stop them waffling when they write the letter.

The letter needs to be properly addressed, they should start of by explaining who they are and what relation they are to you, also how often they see you.

Next, the problems that they see you having with your daily activities and in particular the PIP activities.

The letter needs to be signed and they should attest that the information contained in the letter is correct to the best of their abilities.
