I've just received a date for PIP assessment for this Thursday - seems like very short notice. But I'm very surprised because I have not even returned the Application Form! Surely this can't be right. anybody have experience of this or advice about how I should proceed, please?

I would contact the Assessment Provider, it's unreasonable to expect you to attend without your being able to return the claim form in time, be careful that they accept responsibility for changing the date.
I would also contact the DWP once you have spoken to the AP, so that they know what happened and that it should not count against your two opportunities to attend.
