Requested a recorded appointment for my home visit but the doctor turned up without any equipment because she apparently hadn't been notified. I had the option of waiting another possibly 5 or so months or cancelling the appointment so I chose to go ahead. However later on in the appointment she did see a note "from last year" (I've been waiting for a home visit appointment for several months) requesting the recording.

Just prior to the appointment I tried calling the number provided on my appointment letter to confirm it was to be recorded but could never get through so ended up calling the 0800 number for the Health Assessment service. The person I spoke said he could clearly see I had a requested a recorded appointment so it would be "very, very unlikely" for it not to be recorded but would send an email over to that department just to confirm.

The doctor spoke to my GP after the appointment and there is a consent form in my file which requires my signature which the Dr has filled in herself with "verbal consent provided". She never asked for my permission and a) I was a meter away from her b) my hands work so I'm not sure why she justified needing verbal consent in the first place.

During the consultation she seemed dismayed & confused that my GP hadn't done more to help and insisted that they must not be fully aware of the severity of the situation or forgotten and that I had "fallen through the cracks". My GP in order to cover their own arse has claimed that I have repeatedly rejected their input over the years... needless to say this is very demonstrably, indisputably false since I fought with them to get referred to a specialist out of the area (documented) and reached out for help a number of times (documented) and was receptive to the help I was given despite most of being useless (documented). I never self discharged. I was discharged every single time, by every single specialist/team and not for being non compliant but because they couldn't do anything for me.

Not sure where to go from here and would appreciate any advice.

*Also could someone please tell me if it's possible to get copies of all the communication between DWP, Health Assessment Service & my doctors & everything on my file because I would really like to do this.

I assume this is for ESA?

So you have had an assessment but not yet a Decision?

At this point there is not really anything you can do until a Decision is made.

You can contact your GP surgery for a copy of any information your GP has provided to the DWP.

Sorry yh it's for ESA (reassessment). No decision yet. Is there something I can do about no audio recording equipment being provided & the verbal consent issue?

You can make a complaint about the recording for what it will be worth.

I'm afraid I am confused by the "consent" issue, did you include your GP's name on the ESA50 as a medical contact, if you did then this would give implied consent. If you didn't then I'm not sure how Maximus would have known to contact them.

Is it the consent that you have a problem with or the fact that they contacted them in the first place?
