Hi Gorden,
I am in the support group at the moment, my claim in up for review, i sent the relevant forms back with medical evidence, but have now found out that they have wrote to my GP, and was wondering what is the form they send to the GP requiring, they other thing im concerned about it that the GP that completed the form doesnt really now, me i always see a lady Doctor. What details do they ask from the GP, I do feel quite worried.. Thank You...

Hi Gorden,
Thank you for your reply, im not sure what the doctor wrote on the form, but has i said he doesnt really know me, does the gp report carry a lot of weight on making a decision. i feel very worried about this.. there are thing on the form that the doctor would not know about me...

You should be able to get a copy of the completed form from your GP surgery.
As to what has be written, GPs don't have a good record in responding to DWP requests, the majority of request are not responded to, those that are split fairly evenly between virtually no useful information and a supportive detailed response.
