I have been waiting for my MS nurse to provide a report for me as evidence for my claim - she said she would do it on 14 Feb when i saw her in the clinic. However I have not received anything and she is off sick this week and on holiday next week. The deadline for the form to be sent back is 15 March. Even if i get a 2 week extension she wont be back in time to do the report.

What can I do - i feel that without any evidence at all I may as well not even bother. Help!

Return the form on time and then send the additional information on as soon as it is available, you should also take a copy to any assessment that you might have and ask the assessor to read it before they start the interview and then include it in your file before they return it to the DWP.
