I will be most interested to hear if you get what you need from the GP.
I have experienced the same problems with DWP clearly stating the "onus is on me" to obtain evidence.

You may find this interesting....
When I spoke to PIP advisor they told me they don't generally apply to GP/specialist because
they have to pay a fee. (Obviously they would prefer we pay it., especially if it supports our claim) And their wording is very ambiguous... "may" request... which can leave the claimant expecting DWP/PIP to get the necessary info, especially when GP/specialist says you need an official request.

Don't be put off! Although there is no law stating the DWP/PIP are under obligation to apply for further information, neither is there any law saying we can't.

I will update the post when I hear anything :)

I supplied over 40 pages of medical evidence - none of which was a direct request from me to a consultant or GP.
They were all from requests for my medical records, both hospital and GP.
Yes, they cost me £50 per request, but then you have everything you need from year dot and you can also filter out anything ambiguous or unhelpful for any reason and make the strongest case possible.
I know £100 is a lot of money but its the equivalent of perhaps just less than a weeks enhanced pip on both counts, for instance, so totally worth it

I had a phone call from one of the managers at audiology today...
I was told that the necessary information was being posted to me next week and I would not have to pay for it. The audiologist said he couldn't fill in the gp form, but had written me a letter for the pip M.R. as he felt unable to comment on my day to day experiences... I'll make comment on that when I have seen the letter ;)
I was also told that the hospital trust have decided they will not charge patients for such letters in future when required for DWP claims.

Feeling encouraged!

I had exacty the same problem. No one not even gp would write anything anything.
I finally paid £50 to my gp paractice for copies of my entire medical records.
My daughter sifted through the hudge pile and pulled out at least 40 letters from various specialists that were relevant in some way to the questions being asked , and mentioned various conditions/ diseases I suffer from. Pls MRI/ ct scan / X-ray results etc.
I just sent the lot in! Even very old stuff as it showed how long these things have been going on.
Didn't end up with face to face medical, all decided on medical evidence and form.( this was lb to esa two yrs ago) just done same thing now with dla to pip, but only just posted form with old evidence and newer evidence.

My mother has dementia and she was playing Arcade machines losing thousands,i asked her consultant for a letter saying she was a vulnerable adult,he sent me a letter no problem and it was to whom it may concern. She is now barred out the Arcade shops so tell your consultant he has a care of duty to you.
