PIP1 form -can't access

Hi there,
On your DLA to PIP guide it says. ' 'To see in detail what questions are asked at the initial claim stage, you can download a copy of the PIP1 form from the DWP website at:

but when i attempt this the GOV.UK page says - error 404 'this page cannot be found ' .
Do you have another link or can you direct me to a thread where the details of the questions are discussed?
