Hi.
I'm new to this site.
I recieved my renewal forms for esa,they are asking for proof of illnesses,ie hospital apps treatment ect but unfortunately I haven't really got anything anymore as I have lived with my conditions for many years and are managed by meds and visits to my gp.
I becoming very stressed about this and really don't know what to do.
These forms are so daunting that I've been sitting on them for two weeks and time is running out.
I'm new to this site.

Welcome to the forum, you might want to have a look at the following FAQ which explain where everything is

This assessment is not really any different from your last one.
Although up to date evidence would be better if your conditions are ones that would not be expected to improve then you can use old reports amd letters providing you can show that they are still relevant to how you are today.
What you will have to do is explain your problems in completing the ESA activities in some detail so that those assessing you can understand your difficulties.
You are being assessed against very specific criteria so it is essential that your answers on the form address these requirements, see our ESA Claim guides for the criteria, explantaions of what is required and a strategy for answering the questions based on your conditions and limitations.
