Assessor didn't send piece of evidence to DWP

Falling to pieces over PIP, and now discovered another piece of DWP f*ckery. No idea what I should do with it, if anything.

My assessor didn't ask about suicidal ideation or self harm, and rushed the end of the assessment. I had no time to bring it up, and was worried. I brought 3 new pieces of evidence to the assessment, and one of them is an A&E Liaison Psych Team report from a month before the assessment. It clearly states that the reason I attended A&E was high anxiety and self-harm. I definitely handed him all 3 pieces of medical evidence, and I asked him to read them, and to send them on to the DWP.

In his report he said that I had no suicidal ideation or self-harm. Since he made an enormous number of factual errors in his report, he clearly didn't read my forms or my medical evidence, so I wasn't surprised that he hadn't read that particular document.

I got the DWPs appeal response bundle today. The other two pieces of evidence I brought to the assessment are in the bundle, but the self-harm one isn't! What do I do about this?

I'm writing a Tribunal submission (well, I'm sitting paralysed in front of my computer for hours at a time, dissociating, but the submission is on the screen) and I'm also making a complaint.

I had my partner and a friend with me at the assessment. My friend didn't notice if I handed the assessor the self-harm paper, but my partner clearly remembers seeing it in the assessor's hand. Does this matter? Do I address it?

Thanks. Every little thing is making me panic. Haven't been able to sleep for 56 hours now...

Are you making a submission to the appeal?

Even if you aren't send a copy of the missing report to the Tribunal Service with a covering letter saying that this was provided to the assessor at the assessment but has not been included in the evidence pack provided by the DWP.
