Just got my copy of my assessment.
On the last page it says. For office use only. Then in tge box below it stated three of my medical conditions; not all.
Can please tell me what significance this has?
I am new. How do i get the most up to date subjects in the forums.In anticipation for your help; i thank you.

I'm afraid I don't know what this page is for, it shouldn't have been sent to you. What is included on page 2 of the report, you r conditions should be listed there.
I'm not sure what you are asking for when you say "up to date subjects". The forum displays topics in date order, latest at the top. The Welcome message above explains how to access our guides.

Thank you fir your reply. The link you sent is very usefull as i am not I.T. savvy.
As regards to the other matter. I had my assesment on 4th of April DLA to Pip. Was on high care dla low mobilty. The Descriptors in the copy of the assesment has given me 10 points for daily living and 10 points for mobilty. I feel given my medical conditions and the server impact they have on my daily living. 12 points should of been the minimum for both? Has anyone else got this? only then to find out the DM/CM has made a final decision with both elements Given the higher status of enhanced? Sorry i have add another query.
To try clarify my ponit " For office use only" 30 page report. Page 30 (final page) has Writing time up 45mins. Then the HP Name, Profession Nurse, Date,Then bittom of page. For office use only. Where typed in an oblong box, Three of my medical Conditions. Leaving out my recently diagnose. 2016

I understand what you are saying abut the page marked "office only" but are all your conditions listed on page 2 as this is what the Decision Maker sees?
The first stage to challenging a Decision is for you to request a Mandatory Reconsideration, this needs to be done in writing to the DWP, within one month of the Decision, to the office that dealt with your claim, have a look at our PIP MR & Appeal guide for details of the process, the PIP area also has template letters that you can use to make the request with.

You should contact the DWP for a copy of the assessment report if you have not already done so, I would phone them but again follow up the request in writing. Once you have the assessment report you will have a better understanding of how the DWP Decision Maker has come to their conclusions and will then be able to argue against them.
Your primary task is to show that you meet the criteria, there are many reasons you may have failed, you need to address each of these but don't get bogged down in criticising the assessment report unless you can clearly show that it is incorrect, it is a lot easier to argue the facts of the situation;
"the assessor recorded that I walked 50m, I did but they have failed to document that I had to stop every 10m for a rest due to breathlessness"
than their opinions
"based on my observations of the claimant walking I believe that they can reliably walk more than 200m.
When you have a better idea of the issues with your claim, come back to the forum and we will do our best to help.

Hi Gordon.
Yes all listed on page 2 That i have noted on my Pip2.
I will wait for the DM decision and let you know the out come.
It has been just over 2 weeks since my assesment excluding two weekends. (for your ref)I got my copy of the P4 report within 13 days of the actual f2f.
