Could you help me please. Are the forms for PIP that you need to fill with the help of the sample form on here. May I ask is the sample form of 2015 up to date or is there Anything on there that should be updated please ? Or do I need to get the help of a social worker to fill the form in ?

There is a copy of the current AR1 Review form in the PIP section

The current PIP2 form is not online but our PIP Claim guide (same link) covers the form in detail/

I would always recommend members to get help with completing the forms if they can get trained advice but you may struggle to get help. If you do, remember this is your claim and only you profit of lose by the result so be certain that you are happy with the advice you are receiving.

Do an internet search for "welfare advice" with your postcode, town or county, you can also try any charities that specialise in your conditions.
