esa assessment without esa50 form was created by Alex
hi all, my daughter got a phone call from dwp last week giving her a date for an assessment,but she hasnt been sent any forms.is this normal? she had a face to face last june re her pip claim and was awarded both components. will the pip award go in her favour?, she suffers from anorexia and has been hospitalised on several occasions. her assessment date is tomorrow

The call will have been from Maximus not the DWP.
It is not a legal requirement for an ESA50 to be issued and in cases where the claimant might be considered as being unable to complete the form, it is not issues. However if she feels that it will help her claim if one is complete then she should do so.
She can either download and print a copy

or she can write a letter covering her problems, I would use the same format as the ESA50 to do this.
If there is time send a copy to Maximus, it should be possible to mail them a copy

Take a copy to the assessment and ask the assessor to read it and include it her file before they start the interview and finally send a copy to the DWP office dealing with her claim marked for the attention of the Decision Maker.
You do have the option of cancelling this appointment providing it is the first one offered to give time for the form/letter to be produced but she will need to attend the new appointment, so make sure that it is for a conveninet location, date and time.

Thanks Gordon.just thought it seems a bit strange.having been awarded pip last July that they would assess her for her capability to work

The two benefits are separate, an assessment for one cannot be substituted for an assessment of the other.

just a quick update on my daughters assessment, we were almost at the medical centre when my daughter got a call to say they had to cancel, due to someone being ill. so they gave her a date for next monday so more stress for her. she did ask for a face to face but they said it was only for extreme cases
