Does the same person read each section? I am wondering if I should start every section "Because of my Spinal Bulbar Muscular Atrophy I can't, need help with etc." Or should I just put "Because of my illness...." It looks rather silly as though I am trying to rub it in but wondered if this was the right way to go?

Personally I would repeat the information wherever it is relevant.
As an example someone with grip problems due to Arthritis would likely have difficulties with completing aspects of Preparing Food, Taking Nutrition, Washing and Bathing and Dressing and Undressing. The actual issues might be different but the basic cause is still the same.
This is all about leading those assessing you in the right direction so that they can make the right Decision. Making it easy for them to understand and difficult to argue against.
