I am about to request a mandatory review but there are a couple of things I don'y understand on this form - can you help me?
This remark is made continually throughout the report: 'MSE was unremarkable'
I understand the MSE is mental state examination which I'm assuming is totally based on assessor's observation because I was not asked directly about my feelings, thoughts or mood.
What is 'unremarkable'? She saw nothing therefore I have no feelings, no thoughts and no mood that affects my daily functioning??
FYI: There is the continual remark that I am only on first line antidepressants (which is incorrect information) therefore my mental state and daily activities is/are manageable.
The assessor was a nurse who had no knowledge of diabetes of type 1 or type 2.
There is no mention of me getting very upset about being asked about self-harm and in fact no mention of self-harm in the report.
I am shocked at the inaccuracy of the report! It has had such a devastating impact on my mental health state - my use of helplines such as Crisis Team and Samaritans has increased significantly.
I am battling through trying to write this mandatory request. Last question: Can one get an extension for a mandatory request?

I would take "unremarkable" to mean not sufficient to have an effect on your abilities to complete the PIP activities. So they are not saying that you do not have any mental health issues, just they are not having an impact on your daily activities.
There's no practical way to extend the normal 30 days for an MR to be requested, however, you can make a Late Request providing you explain in your letter why it is delayed and those reasons are accepted by the DWP. Health problems would usually be accepted as Good Cause.
Although dealing with issues in the assessment report is important you will only be successful by showing that you meet the criteria for one, so I would recommend that you spend time going back through your PIP2 and the PIP Claim guide to make sure that you have explained your problems in the correct terms.
