should i order a new pip form? was created by Ree
two questions:

1. i had help filling out the pip form today, but I don't feel like the advisor knew how to communicate my difficulties properly and i am panicking. I didn't expect her to write on my form, i thought we were going to type up answers (as i was planning to write see attached and staple paper for my answers).
Should i request a new form? I'm scared that crossing off each answer on each question and saying please see attached will raise suspicion, or does this not matter?

2. Also, my consultant has said she will write me a supporting statement on my condition and it's effects on daily life, however, it wont be until 3 days before the deadline. I was just granted a deadline extension, should I call and ask for another and explain the reason, or shall i submit without it?

An advisor should know how to complete the PIP2 but this is your claim and only you profit or lose by how your form is completed so if you are not happy with what has been written then certainly consider revising it or replacing the content with your own.

You can request a new PIP2 but I would not wait for it to arrive to continue creating your answers.

It's worth asking for another extension, especially if you ask for another form but if one is not given then then send the form on time you can always send the letter later. If it will help there is a GP Notes document in the PIP area which might help your Consultant with their letter.

So will using a tippex mouse on four lines, of almost all the questions be ok? Or are there rules about this?

I would like to rewrite what she has written.

For example for question 11c mixing with other people an example of the answer given is:

"For the majority of the time, I have poor tolerance of other people and suffer extreme irritability. This causes distress and I feel like I have to escape. I have to keep away from others because it aggravates other condition symptoms."

But viewing the guide on here, I don't think this would score any points as it doesn't follow the format suggested in the guide to give the succinct, in depth knowledge of how my condition affects me.

So whilst what she has written isn't wrong, I just don't think it described my situation how I would've described it and I feel panicked about that staying on the form, even if it is crossed out. Hence why I would like to know if a tippex mouse, or simply cutting out and sticking plan paper over it with strong glue and rewriting it, would be ok?

Providing it is still legible then Tippex will be fine. I would be cautious of sticking paper into the form, I'm not saying not to do it but if you do it needs to be firmly attached on all sides, when the DWP receive the form, the spine is removed and each page is scanned using an automatic feeder, so anything you stick to the form has to be robust enough to survive this.
