New Style ESA: the first form. Can't find guidance

Hi

I am applying for what my Citizen's Advice contact calls 'ESA New Style'. I understand that this is something to do with Universal Credit (though I don't fully understand).

I have a form to fill in for the job centre. It doesn't ask details of my disability - it's more a general introduction form, asking about dates and pensions and if I'm working.

Is there any guidance on how to fill out this particular form? I find forms difficult in general.

The areas I don't understand what I should put:

Part 1 - 'When do you want to claim Employment and Support Allowance' - can I put when I first got ill?
Part 4 - The section on special rules - how would I know what the special rules are?
Part 6 - is PIP a 'social security benefit'?
Part 8 - what is a statutory payment?
Part 11 - I don't have Universal Credit but the question implies I do. ?? 'Your [ESA] will be paid into the same account as your Universal Credit. Do you agree to this?' Do I just put 'yes'? But it doesn't make sense.
Part 12 - there's a section for 'other information' - is there something important I should put in there?

Please help if you can!

1. You need to give a specific date. ESA can be backdated by up to three months providing the time is covered by a Fit Note and there are no reasons why you could not claim ESA at that time, for example you were receiving SSP.

4. This would only be relevant if you have a terminal illness with a expectation of death in the next six months.

8. there's a list on the form

11. I would reply no and supply your bank details or say to pay it to the same account as your PIP.

12. Not unless there is anything else you think is important.
