PIP Form not filled in correctly was created by Claire
Hello,
I recently applied for PIP as I had previously been getting DLA from childhood. I went to an advisor and explained everything about my disability and the help I need day to day. I got the advisor to fill out my form as I was afraid I wouldn’t fill it out correctly and they posted it but I only received a copy of the form after it was posted and upon reading it noticed they have included a lot of things that I didn’t say/ don’t apply to me and left out some important points. I think the form was posted late last week, is it too late to get a new form or should I contact them to explain and provide the correct information.
Also, If I am contacting them should I phone and explain or send a letter and if I send a letter is it better that it is typed or hand written?

Yes, contact the DWP.
Has the date that the PIP2 had to be returned by passed?
If it hasn't then it may be possible for you to receive a new form and complete it as you feel it should have been.
If it has then right a letter with the corrections and send it to the DWP, send it to the Assessment Provider once you have an address and take a copy to any assessment that you have.

Hello, thank you for getting back to me. Unfortunately the date has passed it was on Friday 2nd June. All the letters I have relating to pip come from DFC (department of communities) I am from Northern Ireland, should I send the corrections to them and the DWP? Would the assessment provider be capita?

Ignore the references to the DWP, send the letter to the DFC and Capita.

That’s great, I will send it ASAP and hopefully it will override the wrong/ misleading information in my form
