Covering letter

There's no template for this as it is entirely dependant on your problems.

They need to write a letter with their name and address, how they know you and how frequently they see you, then detail any help that the give you or any problems that they have seen you have with the PIP activities. lastly they need to attest that the information is correct and sign it.
