i have been move from esa to works related 10 months ago ihave worked in same job for 40 years and had to leave because of illness when i went to work related i was told not to come back for a year My has become really bad and i want to apply for esa Can you advise me how

If I understand you are currently in the WRAG but your conditions have deteriorated.
You can ask to be reassessed but you will only be placed in the SG if you can show that you meet one or more of the SG Descriptors, can you do this?
The SG criteria is explained in our ESA Claim guides

Reporting a change will result in a full review of your claim, a new ESA50 and almost certainly a face to face assessment, the Decision that results, whatever it is, will replace your current award.

Who do I apply to and yes my health has become very poorly I ha e developed diabetes .chronic asthma and have a couple of operation on my leg and I am waiting operations on my other leg thank you

I'm afraid none of these will directly qualify you for the SG, it's not your conditions that will be assessed but the limitations that result from them.
If you are sure that you meet the SG criteria then write to the DWP office dealing with your ESA claim, I would send it by a Tracked post so that you have proof of delivery.
