Because he have all his problems such as arthritis and sight problem and hearing loss and he got lost in the place he was very familiar with now he never go out on his own he think that if he go out he is not going to be safe and will get lost and taken to hospital

So you seem to have good evidence to support a higher award, you need to request an MR of the Decision and refer, for example to the CVI certificate that he has for his sight but you also need to make sure that you are using the right terminology when explaining his problems, so for Communicating this means

a. Can express and understand verbal information unaided. 0 points.
b. Needs to use an aid or appliance to be able to speak or hear. 2 points.
c. Needs communication support to be able to express or understand complex verbal information. 4 points.
d. Needs communication support to be able to express or understand basic verbal information. 8 points.
e. Cannot express or understand verbal information at all even with communication support. 12 points.

where “Communication support” means support from a person trained or experienced in communicating with people with specific communication needs, including interpreting verbal information into a non-verbal form and vice versa.

and

“Basic verbal information” means information in your native language conveyed verbally in a simple sentence.
“Complex verbal information” means information in your native language conveyed verbally in either more than one sentence or one complicated sentence.

From your post you are trying to score for the Going Out activity?

This activity looks at three things.

Planning a route - this is primarily a cognitive or sensory (e.g. blindness) activity. You are being asked about the problems you would have with working out how to get from one place to another, you do not need to be able to follow the route that you are planning.

Undertaking a Journey - this is do with mental health issues such as agoraphobia and social anxiety and is concerned with you leaving the house to go somewhere, they will be interested in the things that stop you doing this. You need to show that you would suffer "overwhelming psychological distress" to meet the criteria.

Following a route - This activity about the problems you would have navigating a route. So are there problems cognitive, sensory or mental health issues that would prevent you from doing this? This is different from undertaking a journey, in fact if you cannot undertake a journey then you will not score points for following one and vice versa.

LSt time he got 10 point from
Cannot follow the route of an unfamiliar journey without another person, assistance dog or orientation aid. 10 points. This have to do with him been unable to see the stops or signals and him been unable hearing the message on busy noisy public transport and him been unable to find seat and don't been uble to stand for long cos his pain from arthritis and this make him lost his stop and get lost and been anxious cos he his on his own in the unfamiliar and unknown place

Unable to see the right place to cross the road and to hear the incoming traffic plus slow pace walking is making him and others unsafe

So you have a CVI for his sight, do you have a similar certificate for his hearing?

These are the things that yo need to present as support for his being unable to Follow a Route with an explanation of the problems that he would have with reliably doing so on the majority of days.
