DLA to PIP signatures on form

DLA to PIP signatures on form was created by Cardiff2011
I will be filling in PIP form for friend - physically and mentally he is not able to. I phoned for him for PIP1 and said I would be filling in PIP2 for him. Do I sign form as well as claimant? If so where should I sign? I managed to get 2 week extension as friend not able to see psychiatrist or gp in time for original deadline

If you are using their words on the form then you do not need to sign the form although I would still explain how the form was completed.

If you are using your words then explain on the form why this was done and you should sign the form at the end with the claimant.

Thanks for last answer. We are having problems getting letter/report from GP. He said ask for medical records/notes but delay in surgery providing - deadline for return of PIP2 approaching. Should we say on form that further medical evidence may be sent separately? It depends if there is anything of use in records/notes if we send anything else.

You should not delay sending the PIP form back on time, you can send additional evidence later or take it to any assessment that is booked.
