Attach the pages to the back of the PIP2 using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.

thankyou gordon i will make sure i do that,,,ill be sending it recorded delivery too..can i just ask,i know most likley i will be called for a face to face,,when im there if the interviewer asks me to do things that i think will iether hurt me there and then or will have pain later from it,,things like lifting my leg and balancing,,things like that,,if i refuse to do these things can i get into trouble for it,or will it be ok,to say i dont want to try do it??...thanks gordon,,,,,,,,,john

You can refuse to perform any activity that would cause you unreasonable physical or mental distress if you do then make sure that you explain why you have done so.

thanks gordon i was worried about that becouse i have got 3 different things wrong with my back and muscles in my legs and any sort of preforming for them will hurt and have lasting impact both mentally becouse i dont converse very well with strangers face to face i find it very uncomfertable,,i do have an updated letter from mri scan stating my back problems and i have evedence of my anxieaty and deppresion so all in all i think i will refuse politely to do any sort of preforming,,thank you for your kind reply,,,,,john

hi gordon just a quick question before i post my form off,,,,like i said iv wrote a letter stateing all my ailments and how they make me feel each day,,the trouble is its 3 pages long is that too much information,,i think its all relevant to my claim as its really the only way to get everything out so they can get an idiea of whats wrong with me,,should i send this with side affects of my medication as well,,,what do you think,,thank you in advance i wont post my form untill i hear from you ,,,thank you very much for your help,,,,john

There's no right or wrong length for information, the only thing I would advise is for you to take a second look at it to see whether you can make it more concise, in a former life I used to have to write detailed responses to customer questions and found that my first draft could always be shortened and made more readable.
