PIP form 1043

Hi,I received the PIP 1043 form but it doesn't have any tick boxes for easier,harder and the same just space to write a reply.Is this a new form or are there different variations of the form?

Received same form last weekend. 24 pages but horribly worded. Scary.

the form 1043, is the review form that people have been previously calling AR1

my form had no mention of AR1 on it, but DID say 1043

it is the review form, with the tick boxes (easier/harder/nochange)

if there are no boxes on it then I would think it is more likely a printing error?

My wife has received new Form 1043 no boxes to tick
Every questions on the 12 daily living or mobility activities has the following 3 boxes to fill
Tell us if something has changed and approx when
Tell us how you manage this activity now, including the use of any aids you use
Tell us about any changes to help you need or the help you get from another person

I phoned the DWP about the form having no tick boxes and the lady on the phone said I do have the right form.Maybe they have changed it.
