Hi, can anyone advise, please. I sent my eas renewal form back on the 15 may, My daughter rang the medical centre on Friday on my behalf, to ask where my claim was at.
A very rude man named Carl answered the phone and told my daughter that it was awaiting a date for my medical at one of there centres
.My daughter went on to say, she cant go to a medical centre due to having severe agoraphobia and a letter from her gp was sent with the form confirming this. He very rudely said, she has sent no evidence to support this ??? and agrophobia wouldnt stop her from going for a medical???? In with my form there was evidence of some of my conditions, ane a letter from my gp, she clearly stated a home visit would be needed if a medical was to take place. According to him, I have sent no evidance in with my form. So whats happened to it? my name and national insurance no is wriiten on every sheet of evidence sent. My daughter had to get a bigger envelope because she couldn't fit it in the envelope they provided. I cant remember if she stapled the evidence to my form or not. I rang my doctors to see if the form they had recieved from esa had been sent back, it had so thankfully she has written on there questionaire about the home visit. Can I make a complaint about the rudness of this guy and why is he saying no medical evidence was sent when it was. Have thy lost it, and has anyone had this problem with them.

Did your daughter ring the DWP or the Assessment Provider for your postcode?

In either case, contact the DWP office dealing with your claim to find out what has happened to the evidence that you submitted with your form.

You can send a copy of the GP letter directly to the Assessment Provider but make sure that the letter is unambiguous about your requiring a home assessment any suggestion that there are circumstances in which you can attend will undermine the letter.

my daughter rang the assessment centre. The guy my daughter spoke to had a very poor attitude, when he said agrophobia wouldnt stop me from going for a medical, it was at that point my daughter said that medical evidance had been sent to say a home visted was needed.. he said there was no evidance at all to back this up. My daughter also said that she would get my support worker to send them a letter he said, the letter has to be backed up by either a gp or psychiatrist.? I always thought support workers were able to write evidence. Anyway, I do have a copy of the letter and can send them another copy. My daughter rang them again this morning and a lady answered this time, she said, it is pending, my gp has had a form esa113 to fill in asking questions about my conditions dates they started etc, that was posted end of july. that apparently is what they are waiting for. they would of had that by now surely?
Any the letter my gp wrote is not great detail but better than having nothing. she wrote.
This letter is to confirm that .....suffers from severe agoraphobia, this condition has had a significant impact on her life and on managing her other medical problems.
She is also significantly disabled by a number of other medical problems including COPD, ischaemic heart disease and depression, arthritis, chronic back and neck pain. These conditions have been ongoing for many yrs and are very unlikely to improve.
If you wish to receive further information, i would be happy to help. I don't feel that she is able to attend an assessment and I feel that a home visit for the assessment would be appropriate for her.

As i said not a great letter and not sure if its helpful or not? she doesn't list all my conditions where as my last gp, listed them all and said, it would not be possible for ms .... to attend a medical because of her agoraphobia.

I sent that gp letter also even though it is dated 2015 and differant gp along with the new letter and other bits of evidence although not new evidence and some dated 2013/2015
Im unsure now what to do with the lady this morning saying it is pending the esa113. Should my daughter still ring the DWP do you think about the evidence they should of recieved. It wasnt stapled to the form, just sent in the envolope with the form but all evidance was stapled together. The last time my daughter tried to ring the dwp it had her on hold for over an hr, by which time they were closed. They dont seem to answer the phone ?

I would chase your GP surgery about the ESA113, I'm afraid that many go unanswered and your GP may not complete it in a way that is supportive of a home assessment.

Can you please advise.
I have received a letter from the assessment centre this morning advising that i have to attend a medical at the medical centre. Im in a panic, i dont know what to do? I sent a letter from my gp stating that i have severe agoraphobia, my daughter spoke to them yesterday and there was no mention that they had sent out an appointment, one of the advisers did, however, say that i had no medical evidence to back up my form? and also they have not received the esa113 form back that my gp filled in. This was posted back in the envolope provided. I have checked with the gp surgery yesterday. it was sent on the 3rd august. I find it very strange that they are saying no evidence has been sent. I also sent a gp letter that was also sent with my form, i have written a copy of the letter in my comment above. Im not sure if what my gp has written is enough evidance to warrent a home visit? Yesterday my daughter also contacted the gps and asked them to fax a copy of the esa113She was advised by the assessment centre to do this. Im at a loose end and cant deal with the stress and anxiety this is causing me. Iv had enough. any advice please
on what i should do next

If they are saying that they have not received any supporting evidence about the assessment then you need to chase them again to see whether this is still the case. If it is, then see whether you can supply the information again, perhaps by email or fax so you know that it has been received.


Don't cancel the current appointment unless you have to because it about to happen, you can only cancel once, after that you must attend or you risk your claim being closed.
