If they are saying that they have not received any supporting evidence about the assessment then you need to chase them again to see whether this is still the case. If it is, then see whether you can supply the information again, perhaps by email or fax so you know that it has been received.

Don't cancel the current appointment unless you have to because it about to happen, you can only cancel once, after that you must attend or you risk your claim being closed.

Hi Gordon, i hope you are weel, thank you for your reply.
My daughter rang the gp surgery again this morning informing them that i have have been sent an appointmet at the meidcal centre.They told her if this happens to get back onto them and my gp would write me a letter. MY gp is away on holiday again so, not much chance of that happening. I asked if another gp could do this for me, NO because they dont know me? its on my medical records. I ask if another gp there that i have seen a couple of times on a home visit is she could possibly do me one, she to is away? maybe im getting paranoid but i feel they are fobbing me of. She is however resending the esa113 via fax today. My gp has wrote on that form tht she doesnt feel i could go to the assessment centre for a medical but feels, a home assessment would be more appropriate. I dont feel this is good enough and the lady in admin who sends all these forms back doesn't see how they can help anymore. i stated all i need is for them to say why and how my agoraphobia affects me. My gp knows how my mental health is at the moment as has refereed me back to health team to do a home visit which they are doing but not until Sep. She wouldnt refer me back to them if it wasnt necessary. I couldnt even get her to write me a good evidence letter privately. The gp totally refused and said it wasnt necessary, Im at my wits end. My daughter is ging to ring the assesment back this afternnon when she finishes work to see if they have recieved the fax from my gps. I will pass on the advise you have given and ask if we can send more evidance of. To be honest the evidance i have is quite old and mainly letters from my old gp writing to others about me and what the problems are asking them to see me. I also have a report from the mental health team that was written to my gp with there findngs. I also have an OT assesment report from 2016 but the cit advise told me not to send that in and that we would use it if it came to tribuneral.Im stuck really getting newer evidance because the only time i really see the gp is when she is doing a home visit when i am having problems with chest infections, copd and my angina. I havent visited the the surgery in a very long time.

There is absolutely no value in holding back information for an appeal, your aim is to avoid the need for one, so if you are sitting on additional reports then send them in now, evidence does not "go off" because it has been used.

Hi Goron
I have gathered up my evidence ready for sending, hopefully, this will help me? some is rather dated, some going back to 2012 up to 2016 plus the OT report which is dated 2016. Other evidence i Have is pretty much consistent all the way through but goes back to 2004 which i know is far to old to submit, latter letters is evidence of my heart disease. and COPD and also state i have mobility problems and that have an impact on me. There isnt much i can do about newer evidence, with not attending any of the services anymore due to the agoraphobia i will have my fingers crossed fingers crossed. I am worried tht with going to the cardio in jan this yr this may contradict what my gp and me have said about severe aggrohbia. I have been out twice this yr once to see the cardio and to attend my sons funeral, last yr was i think maybe 3 or 4 times. My gp did talk me round to having an ecg at the surgery which is stated in a letter and other times were the cardio for my heart scan and cardiologist. I have missed my most recent appointments and have had to cancel due to the pure anxiety and stress just thinking about going. They have given me direct a phone no direct to the cardio nurse for when i cant attend. My gp surgery is getting another gp to sign a letter which states I cannot attend a medical due to the agoraphobia and would have to be a home visit. They are faxing that over straight to the medical centre. I have asked for a copy and hope this gp will help more than my present gp? Im not sure if I told you but, the esa113 form that my gp sent, was near to useless, it didn't really contain evidence of my conditions just what i had been diagnosed with. Thank to my gp for all her support. Well, I can do no more. I am worried that with saying I have been to the cardio this is going to sound like a contradiction?

not sure how to explain this in writing, I have had fibro, two types of arthritis and mental health problems for many yrs. along with a few other condition ie, ibs, migrains gerd but not as severe as the first but on going all the same and they do affect me often. copd and heart condition is never going to get any better either. My conditions are ongoing, they could stay the same or get worse but there not going to get better they can either stay the same for awhile or get allot worse. they have worsened over the yrs. The copd is already very severe. there all are ongoing, there has been no improvement, arthritis has worsened over the yrs and according to my old gp and new one, they have both said and writted that, my conditions are very unlikely to improve, not unless they can come up with a miracle cure. The OT has been intouch this afternoon and they are coming to reasses very soon, I wont get the assesment done and get a report intime for thi claim, nor will i get the new referal to the mental health team report through, thats not taking place now until Sep. If i have to have a medical thats fine,MY problem with them is getting them to visit me at home just like the mental health team are doing, m gp does home visits, why on ertth i didnt tell them this i dont know. MY head is all over the place since my son died, i cant concentrat and think about this, its bad enough having to fill them in with yur mental health but grieving on top, why cant the department put it hold just for a short while until you are better abled to deal with the forms. i just want them to come to me and do there medical at my home. MY first esa was dode paper-based, my pip they sent someone out. They have the same evidence now as they did back then. it baffles me. They cant say its normal for someone to go out 3/4 times in a yr. This was only becuse of the emergency with my heart, I didnt attend the appoints straight away, the first time i went i panicked that much that appointment had to be re book. I have a phone no direct to the cardio nurse if i cant manage to get there.This yr I have been out twice. to the cardio once and have misse the last two appointment and to my sons funeral, I wasnt going to go, my daughter and mum calmed me down i was in a terrible state, i only managed the service and had to get back home, i thought i was going to have a heart attack i did have an angina attacks.I should of told the gp all this when she came to visit over my mental health. You dont think when your grieving and having a panic attack because your away from home. Most of the evidence is asking for others to see me because i am terribly troubled by them. most of those appoints that were made i cancelled, i couldn't face going. i went once and never went back again. I even had my methotrexate withdrawn because i failed having a regular blood test. I couldnt have the medication that i needed because of this.Im not quite sure of all the evidence i sent which they said they haven got, so im sending almost all of what i have apart from prior to 2012. the letters are all preety much the same anyway just witten to differant docs. Im sorry i have rabbit on again, my anxiety has rocketed and i dont know when to stop, so sorry about thatt i feel i i am becoming a nuisance. its a good thing i have spelling grammar

hi Gordon sorry to ask another question. you must be getting sick of me., . Would my pip consultation report form PV4 V3 from 2015 be of any use towards evidence for my esa claim? it shows my conditions, symptons and how they affect me along with what assistance i recieve with daily task and also the medical report from the examination i had by the paramedic.There has been no change sicne that report apart from the new diognoses of heart disease.THe report was done by one of there assors. I scored enough point for the enhance on both physical and mobilty. I was jut wondering it this would pass as evidence? It shows a true picture of me although there were a couple of mistakes which i didnt try and get alterd with getting the enhanced rate.
