PIP 2nd Renewal Form PIP1043 Advice Please

PIP 2nd Renewal Form PIP1043 Advice Please was created by Amanda Campion
My wife has been in receipt of PIP for 4 years now, High Rate Care we last renewed In 2016.
We have now had the PIP 1043 form arrive for a review.
My wife's PIP is for several M/H conditions including PTSD , at the 2016 review she was seeing a Consultant Psychologist from a Specialist Trauma Centre, the assessor set her 3 year award for review this year after 2 years saying that they should review to see what difference this had made.
Unfortunately in typical NHS style 3 months later the therapist retired and she was told a replacement hadnt yet been recruited and to see how she went and she could self refer back to the same team.
When she tried this at the start of this year she was told she couldn't do that and had to go back through CBT to be referred back to Complex Trauma Service.
This has meant that unfortunately her condition has remained the same.
The PIP 1043 Form doesn't have tick boxes, it has 3 sections per activity, The first asking if there has been a change which I've answered No in each instance.
The second one asks how you deal with each activity, I didn't want to leave blank even though they have the answers on our previous PIP assessment so have lifted the key things that help for each activity and put them in here.
The third Box asks what changes have happened to the way you do each activity and I have just said there are none.
Q. Is this the way others would approach this form? I haven't been able to find advice relating to this non-tick box form.
My intention was in the extra information to write down what happened with her therapist (long story) and say that she is now finishing a new lot of CBT to enable her to go back to Complex Trauma service.
Q Does that sound like the right way to so this?
We dont have any fresh evidence as nothing has changed but I intend as her main carer to write a letter to enclose, detailing the care and support I give, covering the things I detailed in her last application.
Q is this worth doing
