PIP Tribunal

Hi, I've been a member for almost 2 years and it's the first time I've asked a question so I hope I've done it alright. My daughters PIP review was turned down after a very unfair medical assessment by a nurse. We are appealing this decision through the tribunal. We have just received the DWP's defence papers. On it they state that we did not return the completed review form and that we ignored their follow up letter and they only arranged a medical assessment because they realised she was unable to complete the form herself, all this is untrue as we sent it back well before the deadline with medical evidence. This is the first we've heard about this. They obviously had received the medical evidence as it was noted in their letter when the PIP review and Mandatory reconsideration were turned down. The problem is that they have not included our review form in the bundle or the first lot of medical evidence. Please can you tell us what we should do.
