Where to send ESA50

Hello B&W,

Please could any one assist me. I do not know where to send my form ESA50 on. I have either misplaced it or its missing. The deadline is very soon.

I did phone a lovely man answered by an hour plus twice I was out of it and could not take the information down. But he was kind and sent me on my mobile a DWP free address, its so small the address I thought I would pop in check with the specialists. Would a letter marked with about few words address, reach to ESA with all my documents? I am in excruciating pain and I did not think I could go through another couple of dementing hours waiting for the phone to be answered.

It states on ESA50 form send it to the Health assessment center. But no address.

I have two envelopes not sure if it is the Atos one or Balham Centre, from before?

I would appreciate any help you could provide.
