Some advise what to do

Hi my head is stuck what to do and keeps going blank

I have a pip review fourm. I filled in the first fourm by myself it was very hard and took me 2 weeks to fill. My orginal was messy writing and loads of spelling mistakes

Got the award

Now I dont know weather to tick no change tell them a bit in the boxes how it affects me I have letters from guys and oxleas backing it up

I got one from eating disorder says eating causes distress and he needs engagment to make something small such as a sandwich. He never cooks anything and only eats food that in his safe foods and to eat otherwise he wont eat for a couple days binge out at night and be sick throughout the night. He misses vitual nurition to function.

I called a chairty who get help fill fourms and they said bring it up but im scared it will cause more problems as theses assors seem to pick up abything for a excuse. Like I have problems with people as of years of bullying I mainly stay in and find it very stressful to be around people and talk im always getting told to talk even around family.

Im scared if I get one if theses people they take points off as I engaged with someone to help even through I find it stressful

Can I ask what you would advise as I know by guides and perious assement I get 6 points for food and drinking alltogether

Ok I fill it in but I cant remeber much as memory issues if I pit down I still have problems and a brief summary of what happens aswell as a few letters would that be eniugh

Ok I put down what I think is best and hope for the best I really hate this this makes me more stress and more ill all theses fourns
