hi again. I posted about change of circs a while back and have contacted my local benefits welfare people. They cant even give me an appointment until middle of Jan, for some weeks later. I received the change of circs forms today from dla and they have to be in by 13th Jan. So, can you give me the link for filling out the form DLA 434 Adult....Help with Personal Care...Looking at your claim again. (section 2A) It appears i dont have to fill in the mobility one if i am happy with my present award, which i am. I am terrified of losing my benefits so really need your quidance so i can complete the form to my best ability, thanks alot.

We don't keep copies of claim forms or renewal forms as they are copyright, and I can't find a link to a DLA 434 on the DWP website

Hi Tanis

At the top of the right hand side of the page is the SEARCH button.

Tap in it DLA 434 and you will be be presented with a list of other people who have asked this question before on this Forum.

CrazyDiamond has listed his answer to this question, so scroll down and read it. Consider and read the list.

Try not to be be terrified of losing your benefits.

After 9 years of being on DLA, they recently took the decision to disallow me all my DLA and my £55 pw disability premium has been stopped too. I know things will work themselves out and eventually I'll find a way of coping and resolving all the financial difficulties that have occurred because of this decision.

We have life and we can take delight and enjoy simple pleasures even if we are skint and struggling.

Today it is snowing and outside my garden looks like a picture postcoard. It's beautiful. There are some things money just cannot buy.
