Can anyone point me in the right direction for the form that i can fill in to give to my GP in case the DLA people want a report of my doctor. I know i have seen it on here somewhere but cant for the life of me find it.
I even had the form last year and gave it to my doctor but things have changed a lot since then.

Could it be the one on pages 60 and 61 of the "The Best Possible Disability Living Allowance Claims For Adults On Physical Health Grounds" that you are looking for, in the DLA resources.
