DLA form has only one page for medical evidence

On the DLA claim form the claimant has to fill in loads of pages whereas your mental health nurse only has to fill in one page.

So doesn't that mean that the decision maker takes what you say into consideration much more than anyone else?

The decision maker can call for more medical evidence if they want it.

However, I personally added extra evidence and I'd recommend it. For example, I got statements from people who know me and help to care for me.

add as many extra pages as you need, use paper the same size and wide sellotape they dont get seperated ... make sure you write on one of the original pages that you have added extra pages

I attached about 20 extra pages of my needs and the way my illnesses affected me,

i decided i must give a full account of my daily life,and the DLA unit certainly hadn't provided me with enough space on their page they sent me.
when finished i punched holes in the extra pages and attached them through the holes of the original DLA pagesjust in case they became detached at the DLA unit.Also photocopied everything before sending.
I was awarded HR care indefinitely.[previous award was MR care]

I almost wrote a thesis. I attached 14 extra pages to elaborate the questions. I also attached a diary representing two typical weeks. I cross referenced each question on the attached summary sheets and also on the DLA form giving the page references.

Hubby had a medical at home.

He was awarded HR Mobility and Middle Care.

I think the more information you are able to give the better.

How many pages did you medical professional write?

My mental health nurse only wrote a brief paragraph. No ones mentioned they also get sent a 3 page report questionaire to fill it, but i looked at that in the appeal papers and that was also brief.

So if you write 14 times as much as your medical professional, doesn't what you write have more importance? (at least 7 times more important?)
