quick question re: esa50 form was created by didsmith
hi folks..first post so please be gentle...i have 2 quick questions re: the esa50 form...am expecting mine to drop through the post any minute and am starting to collate my infomation but have noticed that one of the questions early on is for me to provide my p45/p60 from my previous employer......i'm afraid i dont have this...i've been on ib/dla for over 3 years now and my previous employer has long gone and i dont think i was issued one anyway....is this going to be a problem?

second question is if i fail my medical and appeal..i understand i will be placed on a lower rate whilst i appeal....this could be devistating to myself as a lot of my benefits hb/ctb etc depend on being topped up due to the extra premiums,enhanced disability premium etc, i recieve...i take it that i would have to notify the hb/ctb people of the loss of these premiums and suffer greatly whilst i wait for my appeal to be heard...many thanks again

Hi there didsmith - it sounds as if you have downloaded or have a copy of a new application for ESA. The ESA50 limited work capability questionnaire does not ask for any financial information, as you are not making a new claim.

With regard to your premiums are these as a result of getting DLA. If so then these will not change as long as your DLA award does not change.

Have a look in the members area for the guides to completeing the ESA50 form as they are very useful and will enable you to have the best possible chance of getting the correct award.

Also have you been keepimg your GP in the loop in terms of how your illnesses impact on your day to day living? It may be worth while revisiting this with your GP and also explain that he/she may sent a form to complete.

Hi didsmith,

You would not be expected to supply a p45/p60 if you are transferring from ESA.

In the assessment phase you are still entitled to enhanced disability, severe disability, carer and pensioner premiums.
