REQUEST FOR MY PAST MED. REPORT FROM DWP was created by akenasm
Hi there (moderators/forumists),
Well I'm at a fix at the moment. Reason being that I've just started filling in the ESA form sent to me last week today. I had my ESA Exam. last April 2011 at Croydon and very surprised last week ESA form sent to me again to be filled by mid of Sept. latest.
I was put on support group Dec.2009 thru. appeal tribunal and late April 2011 they put me in WRAG. Because of the pressure and axiety I went thru. due to appeal process in 2009, I refused to appeal this time when removed from the S Group. Now my question is, can I ask DWP/DM for the repport made on my appl. of last April 2011? I will be very grateful for any reply from anyone - being a moderator or forumist. Thanks. Michael.

You should be able to request it over the phone or by letter from your local DWP office.
