WP forms are now online and now they are I can ask a question .....like DWP forms when you sign, in this case for example on page 11 it says may ask

all drs ect who have treated you for medical records, ...it also says unlike any DWP form I have seen

"for copies of all medical records (including those in sealed envelopes)" :silly:

www.veterans-uk.info/pdfs/claim_forms/wpa0011a.pdf

What do they mean by sealed envelopes ? I think I know but am not sure

Hi Dave,

The online pdf application form s have been online for a couple of years now. Personally I try not to use them, as if one asks the SPVA for the relevant claim form, then providing it is returned in time, the claim/review can be backdated to the date the form was requested. If one just uses the downloadable ones without requesting the forms first, the date of claim/review etc will be the date the form was received at the SPVA.

Re sealed enveloped, para 4 of the following explains all:

www.nhsconfidentiality.org/?page_id=30

The SPVA being 'ahead of the game' includes it to save having to reprint the forms if, as, or when the 'sealed envelope' system goes live.....

Regards

Pete
