what is a DS1500 report?

hi all

i have just had my DLA renewal forms and when i was working my through it i noticed that they ask for a DS1500 report. what is this for and do i have to have one? as they say i can get one from my doctor. Is this report for claiming under special rules?

Hi sandra DS1500 is pertaining to terminal illness.
