If you feel that your son will not be able to attend a face to face assessment, you should state this in his ESA50, giving reasons.

You should also discuss this issue with his G.P. ASAP, to ensure that you and the G.P. are in agreement, see my post here with respect to the G.P.'s input, and the ESA113 :
