extra information with ESA form

I am working my way through the ESA form and I am finding that spaces for information are not sufficient in size so I have added more information onto separate A4 sheets, what would be the best way to attach with form related to relevent section ?

Hi Arther

I simply write 'see additional sheets' in the boxes and on the extra sheets I have a numbered heading for each activity on the ESA50. For example, '13. Initiating actions'. You can then staple the sheets to the back of the ESA50 and hope that they don't get separated!

Hi from what has been gathered on here, don't forget to put your full name, national insurance number and date of birth on all sheets, you know what the dwp are like! Copy everything, your queestionnaire, all your supporting evdience, and all the additional sheets, I write on the front when i have sent things back although proof of posting certificate attach it to your copy of your form, people here are sendng orginal to atos, and a copy to dm of the local benefits office. When you got the letter saying your benefits will change, the address on the right hand side is the address to send a copy of your questionnaire and supporting evidence to. The orginal goes to Atos. The DM has to take all supporting evidence into acco unt whereas Atos does not, but I would send a copy of supporting evidence to atos too. From what others have said on here it can take 7 days for atos to get your questionnaire as it goes to a post box address. Send it but registered and keep prrof of posting that way if there are any problems such as they deny they have your form you can say well I posted it and I have a number, Good luck :)
