Hi, would like some advice please. Am due to see my gp next week, is it a good idea to give gp info on how things affect me to day to day? I saw care co ordinator at local cmht and he advised to do it, as i gave care co ordinator a list of things i struggle with,
i told care co ordinator i struggle to go to appointmeents as i get very anxious, will only go in the mornng early as few people in the waiting room and, and will only see the same gp in the practice, but even with them i get anxious and lose train of thought, and i get very stressed. He said it was worth writing all that down and giving it to her so she knows what i have problems with. Has anyone else done this? Did it help?

I can barely string a sentance together when I see my GP even though he is great and very supportive. I ALWAYS write everything down... have done for years when going to specialists as well... most of them just put it to one side and started asking me questions :ohmy: so never got to discuss what I came to see them about :angry: :( My GP always reads it then copies it to file which relieves my anxiety a little as at least he knows why I am there :lol:

your gp should be happy to as it saves them a lot of time and gives them the info they need.

Hi Paula

I is definitely a good idea to keep your GP up-to-date with your health problems and its effect on your day-to-day life. Especially if your GP gets an ESA113 questionnaire.

I experience confusional states, so sometimes have to write a detailed list of points. I leave a copy with my GP just as you described. It's a good idea because it provides a clear picture of how you see things.

It's also helpful for your views to be recorded in the file for those later times when you might need to see another GP who doesn't know your case well.

Hi I have just given my g.p a medical overview of all my medical issues and how they affect me so that my file is completely upto date, soome of my consultants don't forward reports very quickly. I didn't make an appointment to see him, i put it in an envelope with a covering letter sealed wrote on the front which g.p it was for and also put private confidentail on the envelope too as I didn't want the receptionist to open it and read some very personal info, that way I didnt have to make an appointment and my g.p now has a fully upto date overview ( or was a month ago). This will also help if my g.p is asked for medical evidence. I also gave him a copy of my completed dla form so he knew that I was also applying for that too.

You can also find the form on here regarding health report from your doctor and maybe print off a few to give out? Could also fill it our before printing it off? Good luck ..Eddie
