Found it, pages 60 & 61 in “The Best Possible Disability Living Allowance Claims For Adults On Physical Health Grounds” and pages 52 & 53 in

If I may just add to Paula’s question. Could the writers (if that’s the correct expression) of the B&W Guides you can trust, compose an ‘Health Professionals Sheet’ for use with ESA claims? I’m sure many of your members would find one invaluable.

Totall agree, although some descriptors for esa and dla overlap it would be helpful to have one for the esa guides. Saw a later forum post and the link for the esa 113 form and page 2 of the descriptors are just tick boxes, and greeneyedlad had a problem where his gp did not fill out page 2 of the form! So when I see gp next week, i am going to give her a list of the things i struggle with all we can do is try right?
