Only just seen this, so sorry not being ignorant at all. It is called "Health Professional's sheet" and is on the bottom somewhere. Surely a Mod can give directions as I have only seen and downloaded and copied the once, as I am new on here too.If not I will endeavour to find it tomorrow.It is a B&W fact sheet. It gives suggestions and you write down the answers to go through with your doctor.
