Thanks Gordon but I don't see a form listed there that has the points scored, isn't there a score sheet now?

Also with the medical reports, do they like during a tribunal stop assessing when they reach the 15 points?

I wish I could give you a definitive answer, but the facts seem to suggest that the DWP have no standards for which documentation to produce when a Decision is made.

For example, Mumsgirl, earlier in this thread has both an ESA85 and an ESA85A, our understanding is that the two reports are mutually exclusive. The ESA65 will tell you what points you scored if fail to score the required 15, but there does not appear to be a form that is used when a claimant does score 15+ points.

We have examples of claimants who have scored, siginificantly beyond the required 15, and others who clearly should have, but have for example, scored points for the physical tests but none for the Mental and Cognitive ones, which they also expected to score against.

You only other option is to make a subject Acess Request for all of the information held on you by the DWP, however, these requests can take upto 40 days to be answered.

There is a guide to SARs in the Other section of the Mebers Only area. See

It does not appear that there is a form reference number allocated to the decision maker's score sheet? However, a score sheet does exist.

You should mention this to the DWP in the subject access request, and ask for the DM's score sheet to be sent to you.

Thank you to the three of you!

It does seemed to be different for all eh?

It does appear from the medical report that I have only been scored on the first two activities so can't help wonder if that was really all or not or if the doctor just stopped there knowing 15 would be score.

Looks like I might need to go the SAR route in order to get all the papers.

BTW what does "the non functional descriptors were not considered for this case as curtailment applied" mean?
