I had a medical for DLA a while ago and got knocked back so rang and asked for a copy of the relevant I gave her a list of all the info i wanted and she said i'll just send you everything.
Well got it today and i'm sure i'm missing some stuff, all i got was Doctors letters from hosp i gave them plus the "DLA Examination Report" which to be honest i can't read as guys writing is so bad so i'm going to have to ask to have it translated into something i can read (anyone know where i stand on that front) plus the form my GP) that was it.
I was under the impression there should be DM info etc on how he made his decision, can anyone let me know what i should do now, I have until early Jan to get appeal form in, so i'm thinking of putting appeal in NOW to get it in and say something on the lines of, not been sent all info and could not read the DLA Examination Report, your thoughts are welcome on this one guys.

Unlike ESA, DLA seems to operate without any specific form numbers on the the various pieces of documentaion, so it's difficult to make a specific request.
That said, you should expect a Statement of Reasons from the Decision Maker, especially where a claim has been refused or lowered.
I'm not sure that the DWP are obliged to provide you with a "readable" copy of the medical report, but we have been discussing this with regard to ESA and it may be possible to write to the Tribunal Service, requesting they produce a typed copy, obviously you can only do this, once they have your papers.
