dla form received is a diff style than before

I have received a diff form to what I have also completed before, have they changed the appearance of them? also I won tribunal in Sept last yr and they awarded me high rate care and mobility until June 2012. Dla say I have to complete new form now and I have 4wks to return etc. New form is Adult New claim and yet I have been in receipt of dla since 2003? am I just being suspicious,my circumstances certainly have not changed, is it poss to repeat the last form as I have a copy of it?

Is this a "renewal" of your award that ends in June 2012?, see here re : this :

Or is it a "review", such reviews are carried out at random, under the : The Right Payment Programme

Hi,I had a successful tribunal last sept and they awarded dla to this June both high rate awards. So I have now received new form to complete 20 wks in advance! an Adult New application form. I have been in receipt of dla since 2003. I have 3 weeks to complete and return and it states very clearly if I dont do the necessary my benefit will cease.

If you have only three weeks to complete the form it sounds likely that you have received this as part of the Right Payment Programme. See

Hello Gordon thank you for that, I have had a look. Trust me to be one of the random picked, is it because I had a successful tribunal I wonder.
The form just seems to be a very short version, more compact and I know from experience these may well be the ones to watch.
I always keep copies and have started to cross reference between this one and my old one.
Like so many sufferers, health hasnt got better they have progressed to more deterioration. But I remain happy, I have you all to thank for your guidanceover the last two years of form filling. Brilliant advice and examples. I even found myself correcting the CAB guy that assisted me with writing last year!

Gordon, I have just read re this subject and I think you may well be right, looks like it. Do I just complete this in the same way I have an ordinary DLA form or is there anything that I need to be aware of. It seems to be a very short form, but you can always read between their lines. My claims are always very indepth, Im just thinking short form prob expected short answers back to them. Am I just being over cautious I wonder?
