Submitting reports re DLA was created by capainFAIRY
I have chronic long term physical and depression illnesses. I receive repeat prescriptions and very rarely see a health professional other than GP because there is nothing else anyone can do for my health, and no changes can be made re my health by anyone.
All I seem to do is send off quite old copies of bits of correspondance that I have, is it advisable to keep sending these off with a dla claim form regardless of their age, eg. industrial injuries 1998, consultants report 2003?

I was in the same boat but have had to re-acquaint myself with my doctor and let them know in detail how my condition affects my from day-to-day. If you don't see your GP then they forget about you and presume that you are well. It is vital to get this sorted out.

Good luck!

Hi CF,

Even if you have older evidence, it may still be relevant, if your conditions have not improved.

It may help to see your G.P., and make sure that he is aware of your limitatations/care needs etc, as he may be contacted by the DWP, see :

www.dwp.gov.uk/docs/medical-reports-completion.pdf

Remember, it is not the medical conditions themselves that would qualify you for DLA, it is more the resulting limiations/care needs, that you need to go into detail about.

There may be some helpful links on this thread, even though it is regarding COPD :

thanks bro58, Im in constant touch with my GP believe me but when these dla enquiry forms are received at the surgery they go into an inbox and are completed by the duty doctor which may not be my GP. Although I have been seen by her for over two years on instruction of the pain clinic the dla enquiry may well not get to her. the last two didnt. And guess what it was the most stressful times Ive ever had. Last time it took over a year, appeals and tribunals etc. It uses my GP's valuable time up writing letters to the dla, and they ignored those because they werent submitted in their timeframe!!
I did however receive high rate care and mobility which anyone looking at my needs etc would see. Believe me thanks to this site I did it all by 'your' book and had some success.

I agree TCP, but I can assure you my GP knows me and mine so very well, she has me down for double appointments and is very thorough.
I just wonder how old reports have to be, I still send some in that well, look older than the history books in the library.
I agree, everyone touch base with your GP, thats what they are there for.
