DOCTORS VISIT

DOCTORS VISIT was created by ROSIE75
I have to go for my medical this Thursday (26th) and I am going to see my doctor tomorrow to try and get him to fill out a ESA113 form so I can have a home visit, as I suffer with severe OCD. The reason that it is so last minute is because the doctor who has virtually always dealt with me from the beginning and knows how bad I am has been on annual leave for the past 3 weeks (sods law!) and is only back tomorrow. If he is willing to do this for me, straight away, (and I know ATOS may not be willing to do it) will this be too short notice for them to arrange a home visit?

Hi D37,

The ESA113, is a form that may be sent to your G.P., by the DWP.

See a copy here :

www.dwp.gov.uk/docs/esa113-interactive.pdf

I suppose that you could try and get you G.P. to fill it in, however he would not be contactually obliged to do this for a patient, only for the DWP.

You could however print it off, and take it with you to discuss it, and in particular how he is going to answer Question 6 of the ESA113.

Hi D37

I should add that if the "Medical " that you speak of on the 26/01/12 Thursday, is your ATOS face to face assessment, you should ensure that you attend, unless you have something in writing from ATOS/DWP, stating that you do not have to.

If you fail to attend, you may be found fit for work, unless you can show just cause for non attendance to the DWP DM. Your benefit may be suspended.

Hi D37,

I have edited your post to lower case, as block capitals come across as shouting to members that use screen readers.

With respect to your question, you should attend unless you have written confirmation that you do not have to, as advised in my previous post.

You can contact the DWP, at the address of the office that deals with your benefits.

See here for contact details of ATOS :

I really would not build your hopes up, on getting the face to face at the ATOS centre cancelled, especially at such short notice, however, trying can do no harm.

ATOS are always most reluctant to give Home Assessments.

Hi D37,

Having just reviewed this thread, I think the first thing that you should clarify with your G.P., is, has he already filled in an ESA113?

If so, how did he answer Question 6?
