Form LT54

The plot thickens, have just got of phone to DWP,(have another name for them) apparently I rang them yesterday requesting LT54, well it certainly was not me, it just makes me shudder, the total hash ups that go on, and I am in the SG for 3 months, according to Ms A, that started August last year, so by my reckoning it ran out November. OH Please let me get fit to return to work, I just despair of the DWP

I was sent a copy of form LT54 with an ESA72 after requesting an ESA72 with a date when a next wca assessment was due. Towards the end of the first page it states when the next assessment is due.
