Crazy Diamond, I agree with what you have said, it could be very difficult for some people to handle, take a copy of the form before you submit it and if you send in additional notes, read and read again prior to the date. On my WCA the HCP missed out a vast amount of the questions asked on the form and just kept asking me what meds I took. Their assessment decision should be interesting. Always ask for a recorded assessment, it will knock their profits to a very small degree and is great evidence for use in an appeal.
