Your original post brings together two seperate situations, which is where I think the confusion is coming from.

First, is whether you need to attend an ESA medical. Whilst a medical is not always required, if one has been requested, you need to attend. There are no conditions that will specifically result in a claimant being excepted from attending one, even someone with a terminal illness may still be required to attend a medical.

Secondly, is the issue of being found to have Limited Capability for Work, and therefore qualifying to receive ESA. It may well be that your condition will qualify you under the Exceptional Circumstances you have quoted in a previous post, this will be decided by a DWP Decision Maker after they receive a medical report from an ATOS "doctor".

Hopefully this will be a simple matter, but I do want to emphasise that you must attend the medical or offer Good Cause why you can't or you risk having your claim closed due to a procedural problem.

I hope this explains it.
