Just over a week to send off ESA50 now was created by Lady_in_bed
I'm panicing pretty badly now as I only have just over a week to post my ESA50.
Its effecting me so much that I keep thinking I should try for an extension is it likely I'll get one or is it too late?
I can't sleep, I have an eye condition so can't look at a computer screen for long so all this extra time trying to fill the form in is causing me horrendous sick headaches and my depression, stress and anxiety is just making me so ill. I cant sit for long so having to sit to do it is taking its toll on my health too.
I don't even know if I am actually capable of completing it now because my health is so affected,
I cant use cab, dial or welfare rights as they don't do home visits and none are near me.
I've only got as far as 'tell us about the help you need to go to an assessment'
I am requesting a home assessment in there, I keep changing it though as I think its wrong and too long and I have just no confidence.
If I put too much information could it go against me so they wont read it. I'm trying to follow the guides but with my issues and the stress I'm finding it so hard.
I need a letter from my gp to support my home assessment but I don't think I will get one as I asked for hospital notes before and they said contact the hospital they dont do it. Should I expect to pay for a note from my gp for this?

How do I get proof of prescriptions list?
And do they contact district nurses?

Hi 'lady', I am sorry to hear that this is taking such a toll on your health :(

It is very unusual to be allowed an extension. Whatever you do, please don't accept the word of anyone on the telephone about this - don't believe you have an extension unless you see it in writing! Bearing in my how little time you have left before you are due to submit your form, I suspect you would not be able to get an extension agreed in time anyway.

Do you have a friend or relative who could help you with the forms? I know that it would mean discussing very personal things, but it would be better than not getting the form in on time. I don't know whether you would be allowed to submit a partially completed form, with a note saying that you were unable to do any more in the time?

As for prescriptions - most surgeries put a list of repeat items on the other side of the prescription sheet - does yours not do this?

Yes, you would need to have backing from your GP if you were to request a home assessment. I don't know whether there would be a charge for this.

Edit - sorry, I didn't answer your question about district nurses. You can put on the form the name of anyone who knows about your condition - but the DWP do not actually have to contact anyone.

You are legally entitled to welfare rights help and if you can supply enough evidence to support a home visit then one has to be made. This is a legal obligation to welfare rights, this may not be CAB, but it will be with another welfare rights community help liason officer.

I always have my prescriptions delivered to my home by the chemists as I don't go out but there is never anything with them. I just sign the green slip at the door. I asked for a list the last time they came but they said the green form is all they have but there was nothing on the back of it anyway so I'm confused as to why I don't get some kind of receipt.
I hate contacting my gp surgery, most of the staff are horrible apart from one and my gp is not really bothered at all. Would they know what I will be asking for when I ask for a note for the home assessment? I don't know what to say

I don't have anyone that can help with the form but I'm the type of person that I have to do it myself because I'll worry about it if I don't. The people that could have helped have a lot going on right now and I can't ask them

I'm convinced I'm going to fail this ESA

I'm sorry PCM, but this information is incorrect.

ATOS are under no legal obligation to provide a home medical, whether they do is at their discretion and is not an appealable Decision.

Whether a claimant has a legal right to welfare rights is debatable, however, this is moot as the limiting factor is availability. Whilst I would certainly encourage all claimants to seek assistance, the depressing fact is that the vast majority will fail, or at best receive a minimal service.

Sorry but I was referring to CAB and the other types of welfare rights agencies. Looking at case history and the welfare rights org, there is a proviso for home visits. However ATOS/DWP is another kettle of fish and I did not direct the comment to them as I know they work in a mysterious way!!!
